First, fill out our registration form. It can be completed on-line, or you can print out a copy and mail or hand-deliver it to our office.

Next, schedule an introductory office visit. This first visit allows us to get to know you and to review your medical history, problems, and concerns.

During your first office visit we'll review your registration form, establish an electronic medical record, and take a photo to add to your chart. The exam will give you a chance to meet your doctor, go over any current medical issues, and receive advice or treatment if you need it. You may also want to sign a release allowing us to obtain your previous medical records filed with another doctor. You will pay for your introductory visit as you leave.
